◇ Purpose and conduct of study
This study aims to develop and evaluate an Intelligent GIS-Based Smart Campus Navigation System with Context-Aware Route Optimization and Hazard Alerts for SLSU-TO. The system is designed to help you locate buildings, offices, classrooms, laboratories, facilities, event venues, and other mapped destinations within Southern Leyte State University–Tomas Oppus Campus. At present, campus navigation mainly relies on verbal directions, physical signage, and your familiarity with the campus.
After using the campus navigation system, you are invited to complete an optional online evaluation questionnaire about your experience. The questionnaire covers indoor and outdoor navigation, text and voice destination search, campus event mapping and QR access, context-aware route planning, and hazard and building access advisories. GPS assists outdoor starting-point selection; indoor guidance uses mapped routes from the indicated building entrance and does not track your indoor position. Rate only the features you have used and select Not Used / Not Applicable for the others. Comments and recommendations are optional. Completing the questionnaire is expected to take approximately 15 to 30 minutes. No additional navigation task is required as part of this survey.
◇ Risk and inconveniences
Possible inconveniences include the time needed to answer the questionnaire, internet connectivity issues, and discomfort when describing your experience. There is also a risk of unintended disclosure of submitted information, which the researchers will reduce through restricted access. Do not include names or other identifying details in comments. The navigation system is an assistance tool and does not guarantee perfect location accuracy or absolute safety. Its routes and advisories depend on available map data and records maintained by authorized personnel. You may stop answering before submission without penalty.
◇ Possible Benefits for the Participants
You may not receive any direct financial benefit from joining the study. However, your participation may help improve the proposed campus navigation system by providing feedback on its usability, functionality, efficiency, reliability, and overall acceptability. The improved system may later benefit you and other campus users by helping you locate campus destinations more easily, navigate more efficiently, and become more aware of hazard-prone or restricted areas within Southern Leyte State University-Tomas Oppus Campus.
◇ Compensation
You will not receive monetary compensation for participating in the study. Your participation is voluntary. You will not receive travel reimbursement, a meal allowance, or any other payment.
◇ Provision for injury or related illness
This survey does not involve medical procedures or require you to walk a route. If you feel uncomfortable while answering, you may stop and contact the researchers. If you have an urgent concern while using the campus navigation system, stop navigating and seek appropriate campus assistance.

◇ Voluntariness of participation
Participation is voluntary and is limited to individuals aged 18 years or above who can provide consent independently. You may decline or stop answering before submission without giving a reason. Your decision will not affect your grades, academic standing, employment, university services, or access to campus navigation. To participate, confirm your age, select the consent checkbox, and click I Agree and Continue. You may contact the researchers about withdrawal after submission; however, a response may not be identifiable for removal because names and account identifiers are not recorded with questionnaire responses.
◇ Privacy and Confidentiality
The questionnaire records your respondent category, campus familiarity, device category, ratings, and any optional comments. It does not request your name, signature, birth date, or account identifier. A consent record includes the consent information displayed, the date and time of agreement at submission, the questionnaire version, and a submission identifier used to prevent duplicate submission of the same form. Responses will be used for this academic study and reported in summary form without identifying participants. Please avoid personal details in free-text answers.
The evaluation response record does not contain GPS coordinates or voice recordings. The navigation application and its hosting environment may maintain separate technical or usage logs, including account-related information, IP addresses, and browser information. Browser-based voice recognition may involve processing by the browser service provider. This form therefore does not promise that all use of the system is anonymous. Evaluation records and exported research files will be accessible only to the researchers, adviser, and designated authorized Evaluation Manager for study administration and analysis. Access to system records is restricted through account permissions.
The evaluation collection period will close one year after the questionnaire is activated for this study. The designated Evaluation Manager will manually hide the questionnaire to stop new submissions. All evaluation responses and their associated consent records will be retained until 12 months after that same activation date, then permanently deleted from the active evaluation database by the designated Evaluation Manager. This is a manual responsibility, not an automatic system action. The project manager will coordinate the removal of downloaded research exports and research backups with their holders and verify completion. Hiding the questionnaire does not itself delete existing records. These periods apply to evaluation records, not to separate operational logs.
◇ Statement of Consent
I confirm that I am 18 years old or above and can provide consent independently. I have read and understood the study information, including the purpose, procedures, potential risks, lack of compensation, privacy arrangements, and limits on removing responses after submission. I know how to contact the researchers if I have questions. I understand that participation is voluntary and that I may decline or stop before submitting without penalty. By selecting the consent checkbox and clicking I Agree and Continue, I voluntarily agree to participate. A PDF copy of this consent information will be available for download for my records.
